How to manage chronic diseases?

There are various options available to you if you suffer from one or more chronic disease including lifestyle changes, medication, allied health services, specialist intervention and ongoing monitoring.

Where to get help?

GP is your first point of contact for initial diagnosis and ongoing management. A well formulated management plan can prevent or better manage the diseases towards a healthier outcome. You might also qualify for bulk-billed sessions as part of ongoing management.
